If you wish to make an appointment for a check-up or consultation with a dentist, please contact one of our receptionists. They will send you written confirmation of your appointment, along with our practice information leaflet. You will also receive a confidential medical history questionnaire and a “Getting to Know You” dental history form; please complete these forms at home and bring them with you to your first visit. Please arrive about 5 minutes earlier than your stated appointment so that we can check all of your details before you see your dentist.
